Room Condition Report form

This is the form students have to fill out to tell the college what's wrong with their rooms. They're usually filled with general descriptions of messed-up or missing items of the room.

At the end of the semester/year, if there are more things messed up or missing when you leave the room, you get charged for the repairs.
